Appointments & Check-In
- New patients: After scheduling, you will receive electronic forms by email or text. We encourage you to complete these before your visit to save time. If you prefer not to, no worries! We have simple paper forms available at our office.
- Arrival: Please arrive at least 15 minutes early to allow time for check-in. We take pride in running on time, and your cooperation helps us provide efficient, attentive care to everyone.
- Cancellations: Please provide at least 24 hours of advance notice when canceling or rescheduling. Missed appointments, late cancellations, and late rescheduling will be subject to fees.
Credit Card on File
Consistent with standard practices at private medical offices, all patients are required to provide a valid credit card to be securely maintained on file. This helps streamline billing and allows our team to focus on patient care. Charges are assessed in accordance with our financial policies and applicable law.

Cosmetic Consultations & Self-Pay Services
Cosmetic consultations will require a fee, which may be waived or credited toward treatment at our discretion. Cosmetic procedures, non-covered services, and self-pay treatments are the patient’s financial responsibility. Fees will be disclosed in accordance with our financial policies, and payment may be required before treatment.
